Joining and leaving the National Health Insurance scheme（国民健康保険の加入・脱退について）

Health insurance schemes provide partial or total coverage for medical expenses incurred in the course of examinations and treatment at medical institutions.
In Japan, there are the following health insurance schemes: "employee health insurance," which people sign up for through their workplace; the "Advanced Elderly Medical Service System," which people aged seventy-five and over sign up for; and "National Health Insurance," which everyone else signs up for.
In this case, "everyone else" refers to people who are not signed up for another health insurance scheme, which includes self-employed people, those working in the agricultural sector, retirees, etc.
If you are not signed up for employee health insurance through your workplace, the "Advanced Elderly Medical Service System," or receiving public assistance, please make sure to sign up for National Health Insurance at the municipality in which you live.

*Non-Japanese residents are also required to sign up for health insurance if staying in Japan for more than 3 months.

Anyone who is not signed up for employee health insurance through their workplace, the "Advanced Elderly Medical Service System," or receiving public assistance is required by law to sign up for National Health Insurance.

After the changeover to the Individual Number card (My Number card)
You can continue to use your paper insurance card (light purple) until July 31st, 2025.
Even after the changeover to the Individual Number card, please do not throw away your paper insurance card until July 31st, 2025.
If there are any changes to your details after the changeover to the Individual Number card (from December 2nd, 2024 onwards):
・If you do not have an Individual Number card: you will be issued a Certificate of Eligibility*1.
・If you have an Individual Number card: you will be issued a Notice of Eligibility Information*2.

*1 Certificate of Eligibility: You can show this to a hospital or other medical institution to receive treatment covered by health insurance.
*2 Notice of Eligibility Information: You can use this to check the insurance information that has been registered.

If any of the following 3 items apply to you, you need to notify the National Health Insurance Section at the ward office of the ward in which you currently live within 14 days of applicability.

• If you move to Yokohama from outside the city

• If you stop receiving public assistance

• If you have a child

• If the health insurance cardholder dies

• If you move in from another ward in Yokohama City

• If you relocate within your current ward

• If the head of household changes

• If a household is split up or joined together

• If your name changes

• If you are going to live outside Yokohama City for study

• If you have lost or damaged your health insurance card

   *Please obtain a Certificate of Loss of Eligibility for health insurance from the workplace where you resigned or from the place that issued your insurance card or certificate of eligibility, etc.
   *In the event you do not have the prescribed form for the certificate of loss of eligibility for health insurance, please use the Certificate of National Health Insurance Acquisition/Loss of Eligibility Form.（PDF：430KB）

   *If you are using your Individual Number card, you can complete the procedure by showing the My Number Portal screen together with your Individual Number card.
2. Your current National Health Insurance card or Certificate of Eligibility
   *If you have moved or if the details on your Certificate of Eligibility, such as the name of the head of the household or the address, have changed, please bring the Certificate of Eligibility for all members of your household.

From the date you joined your workplace's health insurance scheme or National Health Insurance Society onward, you are no longer entitled to use your Certificate of Eligibility, etc. for City of Yokohama National Health Insurance.If you do, you may be asked to pay back any medical fees that the City of Yokohama has covered.

   *If you have moved or if the details on your Certificate of Eligibility, such as the name of the head of the household or the address, have changed, please bring the Certificate of Eligibility for all members of your household.

From the day you move out of Yokohama City onward, you are no longer entitled to use your Certificate of Eligibility, etc. for City of Yokohama National Health Insurance. If you do, you may be asked to pay back any medical fees that the City of Yokohama has covered.

2. National Health Insurance card or Certificate of Eligibility that you have been using up until now
   *If you have moved or if the details on your Certificate of Eligibility, such as the name of the head of the household or the address, have changed, please bring the Certificate of Eligibility for all members of your household.

*Please submit notification at the ward office of the ward in which your new home is located.
